From a minor fender bender to a major car accident, the State of Oregon requires your auto insurance to provide you with Personal Injury Protection. Within that requirement, you are provided chiropractic treatment and massage therapy free of charge for up to 1 year.

WORKER'S COMP
We accept workers compensation insurance if you have been injured on the job. Here are a few details you should know first:
Although it can be complicated and difficult to navigate, your workers compensation insurance will pay for treatment if you are hurt from a job injury.
You have already been seen by a doctor for this claim: You will first be required to obtain a referral letter from your Primary Care Physician. That letter can be faxed to our office and must list Elevation Chiropractic & Rehabilitation as the provider you are being referred to.
You have NOT seen any doctor for this work injury: Call our clinic and get scheduled for your initial appointment right away!
